Chronic Condition Management
Managing an ongoing health condition often requires care that extends beyond a single appointment.
Eligible patients may benefit from a GP chronic condition management plan. This provides a structured way to discuss health goals, review current needs, and coordinate appropriate care. Referrals to eligible allied health services may also be available when clinically appropriate.
Conditions that may benefit from ongoing management include diabetes, hypertension, osteoporosis, arthritis, and chronic pain. Your GP can discuss whether a plan is suitable for you.
- Individual care planning
- Health goals and regular reviews
- Support for ongoing conditions
- Allied health referrals when eligible
Health Assessments for People Aged 75 and Over
Eligible patients aged 75 and over may be able to access a comprehensive health assessment once every 12 months.
This longer appointment looks at your health more broadly and may include a review of medications, mobility, nutrition, safety at home, and the support available to you. It can help identify emerging concerns and support greater independence and wellbeing.
- Annual comprehensive assessment
- Medication and mobility review
- Nutrition and home safety
